During the summer of 2011 news of a new strain of MRSA (Methicillin-resistant Staphylococcus aureus) found in milk in the UK made considerable headlines. 

 

Although I wrote about it in UK: `New MRSA’ Strain Spreading, the real expert in all things antibiotic resistant in the blogosphere is Maryn Mckenna on her Superbug Blog, who provided in-depth coverage in More MRSA, in milk: A new strain in cows and humans.

 

Today Maryn is back with another report, this time on the detection of the ST398 strain of MRSA, recently detected in Milk produced in England.

A Health Crisis In Slow Motion

Although truly the miracle drugs of the 20th century, antibiotics are a fragile armamentarium, and over the past 70 years we’ve learned just how quickly bacteria can evolve to evade their antimicrobial effects.

 

Antimicrobial resistance is a huge and growing threat to public health – perhaps the greatest threat of all

 

Yet it is largely unappreciated by the public because its progression has been gradual, the loss of antibiotics incremental, and so far at least . . . there have always been replacement drugs available when an antibiotic has failed.

 

But the number of new antibiotics in the pipeline are desperately few, and the frequency of newly emerging resistant bugs has increased in recent years. 

 

Last March, the Director General of the World Health Organization, Margaret Chan - in a keynote address to the Conference on Combating Antimicrobial Resistance in Copenhagen - painted a bleak picture of the future of antibiotic availability if action is not taken.

 

The D-G’s entire remarks may be viewed on the WHO’s website at Antimicrobial resistance in the European Union and the world, but I’ve excerpted a few choice statements below.

 

If current trends continue unabated, the future is easy to predict. Some experts say we are moving back to the pre-antibiotic era. No. This will be a post-antibiotic era. In terms of new replacement antibiotics, the pipeline is virtually dry, especially for gram-negative bacteria. The cupboard is nearly bare.

A post-antibiotic era means, in effect, an end to modern medicine as we know it. Things as common as strep throat or a child’s scratched knee could once again kill.

 

Some sophisticated interventions, like hip replacements, organ transplants, cancer chemotherapy, and care of preterm infants, would become far more difficult or even too dangerous to undertake.

 

This week (Nov. 12th-18th) marks the CDC’s fifth annual Get Smart About Antibiotics Week, which coincides with observances in many regions around the world, including  European Antibiotic Awareness Day, Australia's Antibiotic Awareness Week and Canada's Antibiotic Awareness Week.

 

The theme of this campaign is the smart usage of antibiotics, and educating the public on the fact that these drugs won’t cure a viral infection, and their overuse leads to resistance.

Get Smart. Take a look at this chart to find out which upper respiratory infections are usually caused by viruses — germs that are not killed by antibiotics. Talk with your doctor about ways to feel better when you are sick. Ask what you should look for at home that might mean you are developing another infection for which antibiotics might be appropriate.

 

For some ideas about what to do for these types of illnesses without taking antibiotics, the CDC has a Symptom Relief webpage.

 

Still, many people expect an antibiotic when they go to their doctor with a respiratory infection (and many doctors continue to prescribe them).

The problems of antimicrobial resistance go far beyond the overuse, or misuse of antibiotics by the American public. Other threats include:

 

 

 

It will take a multi-pronged, international effort to slow the growth of antibiotic resistance. Agencies like the FDA, CDC, WHO, and ECDC are working towards finding solutions, but there are many competing interests and much bureaucratic red tape.

 

Progress is often slow. 

 

Other than supporting the wise use of antibiotics internationally, admittedly as individuals we can’t do much about the use of antibiotics in India, or on the farm, or sold over the internet.

 

But we can be smart about how we use these drugs. Part of the solution is not being part of the problem.

 

We either exercise proper stewardship over the remaining drugs we have (and hope that new ones can be developed) or we face a very grim future where minor infections - once easily defeated -  will once again claim millions of lives.

    A topic that Maryn McKenna and I have both blogged about at length over the past year – Increasingly Drug Resistant Gonorrhea - was the subject of a major announcement out of the CDC yesterday.

     

    In the wake of rising treatment failures with the existing treatment protocol, Thursday’s MMWR contained new guidelines on the treatment of gonorrhea, making the oral antibiotic cefixamine no longer the first line drug of choice, and substituting injectable ceftriaxone along with oral doses of either azithromycin or doxycycline.

    History has taught us – that given enough time – bacterial pathogens often manage to evolve resistance to whatever antibiotics we use against them.

     

    With few new classes of antimicrobials in the developmental pipeline, the fear is that a few years down the road, our ability to treat even the simplest of infections may be compromised.

    Video: Maryn McKenna On Antibiotic Resistance

    Short of seeing a 1918-level or worse pandemic, I’ve little doubt that the rise of antibiotic resistant bacteria will be the number one public health story of the next couple of decades.

     

    While MRSA was the big concern 3 or 4 years ago, today we are seeing the rise and geographic spread of NDM-1, extremely drug-resistant TB (XDR-TB), along with an expanding array of Carbapenemases  – bacteria that are resistant to the Carbapenem class of antibiotics.

     

     

    Journalist, author, and everybody’s favorite scary disease girl Maryn McKenna produced a terrific book on the subject of antibiotic resistance in 2010, called SUPERBUG: The Fatal Menace Of MRSA

     

    You can read my review of it HERE. Maryn is also the author of Beating Back The Devil, the inside story of the CDC’s Epidemic Intelligence Service.  

     

    All of which puts Maryn on the top of the list of people you’d want to include in any documentary on antibiotic resistance. As it happens, UJI Films is working on exactly that; a film scheduled to be release later this year called RESISTANCE.

    When I saw the reports on Friday about India’s Health Ministry’s attempts to downplay reports of totally drug resistant tuberculosis, I was immediately reminded of the old joke about the doctor who couldn’t cure you, but for a small fee he’d touch up your X-rays.

    Resistant TB: The Limits Of Surveillance & Reporting

    With practically any disease you’d care to mention the number of cases that we see, and are counted, generally represent only a small subset of the total.

     

    As the chart from the CDC below indicates, public health officials are usually only aware of the `tip of the disease pyramid’.

    Many people suffering an illness do not become sick enough to seek medical care (or may live in an area where such care, or testing is unavailable). Those that see a doctor may not be properly diagnosed.  And those that are diagnosed may not be reported to the health department.

     

    When the World Health Organization announced the detection of the 571st human H5N1 infection, or the ECDC reports that 4,200 people were infected by the E. coli O104:H4 outbreaks in Germany and France, everyone in the public health arena understood that those numbers are unlikely to include all of the cases out there.

     

    Reporting and surveillance – even in technologically advanced regions like the United States and Europe – are simply not that good. 

     

    So when we learn – as we have this week – of a dozen cases of `totally resistant tuberculosis’ in India, it’s a pretty fair assumption that there are more cases out there, as yet undetected.

     

    The question is:  How many?

     

    While the answer to that question remains elusive, we’ve some troubling hints today from an article that appears in the Indian Express.  

     

    The story revolves around an interview with Dr John Kenneth, Professor and Head, Infectious Diseases, St John’s Research Institute. The article can be accessed at the following link:

    While the headline, that this new form of resistant TB `may be more widespread’ is practically a given - what is worrisome are the percentages being reported.

     

    Dr. Kenneth states that his group has unpublished data showing that out of one hundred TB patients they had randomly selected for drug susceptibility studies, six were found to be `totally drug resistant’.

     

    Additionally out of these 100 cases, 30 were found to be multi-drug resistant (MDR), and 13 were defined as being extensively drug resistant (XDR).

     

    Dr. Kenneth admits that there may be some selection bias in the 100 patients selected for this study, and that all cases tested were randomly selected from among TB patients at only one treatment facility.

     

    Until this study is published, we are lacking details on how the screening for drug resistance was done, and exactly how `totally drug resistant’ was defined. This is an issue that the ECDC brought up earlier this week, when they cautioned against the use of the term TDR-TB (see ECDC Comment On Drug Resistant TB In India).



    Whether these results reflect the prevalence of resistant TB across a wider region is – for now, at least – unknowable.

    The World Health Organization  released a new report and a factsheet on MDR-TB & XDR-TB in advance of this yearly event on the the status of Tuberculosis around the world and the progress being made in its control.

    While progress has been cited in the global fight against tuberculosis, the addition of a new supposedly `totally resistant’ form of TB to the mix complicates matters enormously. 

     

    Yesterday the WHO published the following notice:

     

    Tuberculosis that is “resistant to all drugs”

    13 January 2012 | Geneva | WHO's Stop TB Department has just published a briefing note with "Frequently Asked Questions" to provide information on tuberculosis that is resistant to all drugs.

     

    In 2006, the first reports of extensively drug-resistant tuberculosis (XDR-TB), an even more severe form of drug resistant TB than multidrug-resistant TB (MDR-TB), began to appear. Within a year of the first reports of XDR-TB, isolated cases were reported in Europe that had resistance to all first-line anti-TB drugs (FLD) and second-line anti-TB drugs (SLD) that were tested. In 2009, a cohort of 15 patients in Iran was reported which were resistant to all anti-TB drugs tested. The terms “extremely drug resistant” (“XXDR-TB”) and “totally drug-resistant TB” (“TDR-TB”) were given by the respective authors reporting this group of patients. Recently, another study has been published that describes 4 patients from India with resistance to all tested drugs.

    Referral: Maryn McKenna On Regulatory Obstacles To Fecal Transplants

    Maryn has a fascinating article this morning on her Superbug Blog on the regulatory obstacles that are hindering what appears to be a safe, cheap, and extremely effective cure for often intractable and devastating C. diff infections; Fecal Transplants.

     

    First a link to Maryn’s article (which goes along with a series she is writing for Scientific American), and then I’ll return with a brief anecdote.

     

     

    Fecal Transplants: They Work, the Regulations Don’t

     

     

    A little over 40 years ago (1970 to be precise), I read a book that had a profound effect on my future. It was Dr. William Nolen’s The Making of A Surgeon, which recounted his surgical internship at Bellevue Hospital during the 1950s.

     

    It is what inspired me to become a paramedic.

     

    One of the stories he relates in this book was of a patient in the surgical ward suffering from uncontrollable diarrhea due to what was likely C. diff, who was at genuine risk of dying.

     

    Nolen and two other interns were at wit’s end, trying to treat this person, until one of them reasoned that if they could introduce good bacteria from a healthy patient into this patient's gut, they might effect a cure.

     

    What followed next essentially involved chocolate milk, a `secret ingredient’, a lot of stirring, and no informed consent.

     

    When their resident found out what they’d done, he went ballistic, and for a time all three interns thought their careers were ended.  They waited in fear for days for the patient to die from their ill advised treatment.

     

    As it turned out, the `cure’ worked.

     

    Although the delivery method (and donor screening) used by Nolen and his cohorts needed substantial upgrades, it is a bit amazing that 60 years later this technique hasn’t become routine.

     

    While it is a bit dated, I highly recommend Dr. Nolen’s book to anyone who has an interest in the hectic, and often hilarious, learning curve of medical interns.

    World MRSA Day: October 1st, 2011

    While some reductions in the spread of Hospital Acquired Infections (HAIs) have been reported in the past couple of years, MRSA remains a serious public health threat and is increasingly is being found outside the hospital environment.

     

    Although the true burden is likely higher, in 2008 the ABCs (Active Bacterial Core Surveillance) report estimated that MRSA was responsible for nearly 90,000 serious infections and more than 15,000 deaths in the United States each year.

     

    In 2009, Jeanine Thomas - who created the MRSA Survivors Network - pushed for the creation of World MRSA Day, and this year that observance is set for October 1st.

     

    This year, the global theme is "The MRSA Epidemic - A Call to Action.", and the kickoff event will be live-streamed from Loyola University on the internet tomorrow morning.

    This outbreak is unique, in that Listeria has never been found in cantaloupe before, and exactly how this contamination occurred is still unknown.

    McKenna Wins 2011 Science In Society Award For Superbug

    When I reviewed Maryn McKenna’s Book Superbug: The Fatal Menace of MRSA I wrote:

     

    In moments of quiet reverie I often find myself wishing I could dance like Astaire, or maybe sing like Sinatra, or perhaps compose like Gershwin.

    But in truth, I’d happily settle for being able to write about science and medicine as well as does Maryn McKenna. (see my full review)

    I’m happy to report that others feel the same way about Maryn’s talents. It has just been announced that Maryn will receive the NASW 2011 Science in Society Journalism Award for her book Superbug.

     

     

    2011 Science in Society Awards

    The winners of the 2011 Science in Society Journalism Awards, sponsored by the National Association of Science Writers, are:

    • In the Book category, Maryn McKenna for her book Superbug: The Fatal Menace of MRSA (Free Press).

    • In the Science Reporting category, Katy Butler for her New York Times Magazine article, “My Father’s Broken Heart”

    • In the Science Reporting for a Local or Regional Audience category, Barbara Moran for her Boston Globe Magazine article, “Power Politics.”

    • In the Commentary or Opinion category, Charles Homans, for his Columbia Journalism Review article, “Hot Air.”

    Winners in each category receive a cash prize of $2,500, to be awarded at a reception on October 15, 2011, during the ScienceWriters2011 meeting taking place this year in Flagstaff, Arizona.
