IAEA Preliminary Assessment Of The Fukushima Disaster

Last week IAEA nuclear experts from 12 countries (Argentina, China, France, Hungary, India, Indonesia, Russia, South Korea, Spain, Turkey, United Kingdom and the United States) arrived in Fukushima, Japan to conduct interviews with local officials and to visit nuclear facilities, including the earthquake and tsunami damaged Fukushima Daiichi power plant.

 

They have produced a brief (3-page) preliminary assessment of the safety issues related to the Fukushima nuclear crisis. A full report will be delivered to the Ministerial Conference on Nuclear Safety at IAEA headquarters in Vienna in about 3 weeks.

They describe the level of cooperation, and access, their expert mission received as being `excellent’, and the response of the workers at the stricken plant as `exemplary’ and `brave’

 

They also reassure that: To date no health effects have been reported in any person as a result of radiation exposure from the nuclear accident.

 

From today’s  UPDATE: IAEA Fact-Finding Team Completes Visit to Japan (1 June 2011) on the IAEA website, we get an overview of the expert mission, and a synopsis of their findings:

 

 

The expert team made several preliminary findings and lessons learned, including:

  • Japan's response to the nuclear accident has been exemplary, particularly illustrated by the dedicated, determined and expert staff working under exceptional circumstances;
  • Japan's long-term response, including the evacuation of the area around stricken reactors, has been impressive and well organized. A suitable and timely follow-up programme on public and worker exposures and health monitoring would be beneficial;
  • The tsunami hazard for several sites was underestimated. Nuclear plant designers and operators should appropriately evaluate and protect against the risks of all natural hazards, and should periodically update those assessments and assessment methodologies;
  • Nuclear regulatory systems should address extreme events adequately, including their periodic review, and should ensure that regulatory independence and clarity of roles are preserved; and
  • The Japanese accident demonstrates the value of hardened on-site Emergency Response Centres with adequate provisions for handling all necessary emergency roles, including communications.

 

While cloaked in the polite language of diplomacy, the bottom line is that what disaster planners and nuclear regulatory agencies assumed to be a `worst-case scenario’, and planned for  – a 5.7 meter tsunami – turned out to completely inadequate on March 11, 2011 when a series of 14+ meter tsunamis slammed into the Fukushima nuclear power facility.

 

It has recently emerged that this wildly optimistic worst-case disaster scenario’ came from a 1-page, decade-old memo generated by Fukushima plant operators, and that it provided little in the way of scientific data to back up their assessment (see AP article AP Exclusive: Fukushima tsunami plan a single page).

 

 

Those expecting any sort of critical exposé here will find this report lacking. Given Japan’s level of social, political, and economic uncertainty in the wake of this three-pronged disaster, the IAEA is obviously (and understandably) treading carefully here.

 

Prime Minister Naoto Kan faces a no-confidence vote this week, the Japanese economy is reeling, public confidence in TEPCO and the Japanese government’s disaster response is waning, and the Japanese people are enduring a collective tragedy almost beyond comprehension.

 

So diplomatically, this report avoids assigning blame or directing criticism, so as not to aggravate what is obviously a precarious situation.

 

A full post-mortem analysis of what happened - and what continues to transpire as crews attempt to contain this nuclear crisis - will no doubt have to wait until the emergency has passed.

 

And given the size and scope of this disaster, that could be months or even years from now.

IAEA To Send Fact Finding Mission To Fukushima

The UN’s Atomic Energy Agency (IAEA) will dispatch an expert fact-finding mission to Japan on May 24th to  make a preliminary assessment of nuclear safety issues at the Fukushima Daiichi nuclear power plant following the March 11th earthquake and Tsunami.

IAEA Sends International Fact-finding Expert Mission to Japan

17 May 2011 | The International Atomic Energy Agency will dispatch an international expert fact-finding mission to Japan.

 

Based upon the agreement between the IAEA and the Government of Japan, the mission, comprising nearly 20 international and IAEA experts from a dozen countries, will visit Japan between 24 May and 2 June 2011. Under the leadership of Mr. Mike Weightman, HM Chief Inspector of Nuclear Installations of the United Kingdom, the mission will conduct fact-finding activities at Tokyo Electric Power Company's Fukushima Dai-ichi Nuclear Power Station (NPS) site and in other locations.

 

The expert mission will make a preliminary assessment of the safety issues linked with TEPCO's Fukushima Dai-ichi NPS accident following the Great East Japan Earthquake and Tsunami. During the mission, areas that need further exploration or assessment based on the IAEA safety standards will also be identified.

 

In the course of the IAEA mission, the international experts will become acquainted with the Japanese lessons learned from the accident and will share their experience and expertise in their fields of competence with the Japanese authorities.

 

Mr. Weightman will present the mission's report at the Ministerial Conference on Nuclear Safety organised by the IAEA in Vienna from 20 to 24 June 2011, as an important input in the process of reviewing and strengthening the global nuclear safety framework that will be launched by the Conference.

 

Updates on the situation at the damaged Fukushima facility are coming less frequently from the IAEA, with the last one issued on Friday May 13th.

Each day, however, seems to bring new revelations on the extent of the damage to the fuel rods at the damaged reactors and revised plans to rectify the crisis.

Japan Revises Plan to Bring Fukushima Reactors Under Control

Tokyo Electric will step up measures to prevent groundwater contamination at the stricken Fukushima nuclear-power plant amid worries that highly radioactive water is leaking from the core of at least one reactor.

 

Company officials say work will begin immediately to build a drainage system that will pump the water to a reprocessing facility where much of the radioactivity can be removed. It will then be re-circulated through the cooling system.

 

Recent data analysis concluded a meltdown of nuclear fuel likely occurred within a day of the March 11 earthquake and tsunami that devastated the region. The melted fuel is thought to have created holes allowing water to leak from the center of the reactor.

 

The work is one of the new measures in Tokyo Electric's plan for taking the Fukushima Daiichi plant from crisis to stability. Other measures will be visually apparent at the plant in the coming months.

That more than two months later the story continues to evolve regarding the hours and days immediately following the disaster should come as no great surprise.

 

As I wrote in The Fog Of Disaster Reporting, two days after the earthquake struck:

 

Early reports from the ground during any major disaster are usually fragmentary, often misleading, and occasionally just downright wrong.

 

Over the past 60 days, we’ve seen our share of `erroneous’ reports.

 

Obviously, it is important to learn exactly what happened at Fukushima, what responses were mounted, and what worked . . .  and what didn’t.

 

While one always hopes another disaster of this sort never happens, there are lessons to be learned here. Particularly from the things that did not work as planned.

 

Lessons that could save lives the next time the `unthinkable’ happens.

Reuters this morning is reporting that the Japan has called for the halt of the sale of potentially radioactive Iodine contaminated foods produced in the Fukushima region.

Contamination in Food Products around Fukushima

The Japanese Ministry of Health, Labour and Welfare has confirmed the presence of radioactive iodine contamination in food products measured in the Fukushima Prefecture, the area around the Fukushima Daiichi nuclear power plant. According to the latest data, the food products were measured from 16-18 March and indicated the presence of radioactive iodine. To date, no other radioactive isotopes have been shown to increase in the analysis of food products around Fukushima.

 

Though radioactive iodine has a short half-life of about 8 days and decays naturally within a matter of weeks, there is a short-term risk to human health if radioactive iodine in food is absorbed into the human body. If ingested, it can accumulate in and cause damage to the thyroid. Children and young people are particularly at risk of thyroid damage due to the ingestion of radioactive iodine.

 

Japanese authorities have implemented two critical measures to counter the contamination of food products by radioactive iodine. First, on 16 March, Japan's Nuclear Safety Commission recommended local authorities to instruct evacuees leaving the 20-kilometre area to ingest stable (not radioactive) iodine. As an established method of prevention, the ingestion of stable iodine can help to prevent the accumulation of radioactive iodine in the thyroid. Stable iodine pills and syrup (for children) have been made available at evacuation centres. Second, the Ministry of Health, Labour and Welfare has ordered a stop to the sale of all food products from the Fukushima Prefecture.

 

The IAEA has passed this information to the Food and Agricultural Organization of the United Nations (FAO) and the World Health Organization (WHO) and will continue to report on this development.

 

According to materials on its website, the FAO is prepared to respond upon request from the Government of Japan in the following areas:

  • - assessing radioactive contamination of the agricultural environment, especially foods
  • - providing technical advice and determining appropriate medium- and long-term measures for agriculture -- including soil, land, forests, crops, fisheries, animal health and welfare and food safety
  • - facilitating international trade of foods, including agricultural produce

 

The IAEA continues to gather information on this development and will report further as events warrant.

Japan Raises Severity Of Fukushima Crisis

In the past hour (5am EDT) reports have crossed the wires that Japan’s nuclear safety agency has raised the severity level of the nuclear crisis at Fukushima from a 4 (local impact) to a 5 (wider impact).

 

This does not seem to signify any abrupt change in the situation on the ground in Fukushima, but rather appears to be an admission of what most observers already strongly suspected; that the seriousness of Japan’s nuclear reactor crisis has been greater than they’ve been publicly stating.

This comes literally just hours after the head of the IAEA, Yukiya Amano - upon his arrival in Japan to oversee that agency’s monitoring of the situation – called upon the Japanese government to be more forthcoming with information on the crisis.

TOKYO, March 18 (Xinhua) -- International Atomic Energy Agency (IAEA) chief Yukiya Amano on Friday urged Japan to provide more information on its "extremely serious" crisis, as the battle to regain control of a failing power plant in northern Japan enters a second week.

 

During talks with Prime Minister Naoto Kan and Foreign Minister Takeaki Matsumoto, the United Nations' nuclear watchdog chief said that the central government needs to make its information regarding the crisis at the Fukushimi No. 1 power plant, 240 km north of Tokyo, more readily available to the United Nations agency and the broader international community.

 

"There is the opinion in the international community that more detailed information is needed," Amano was quoted as telling the Japanese leader.

Although often cloaked in `diplo-speak’, the growing frustration of agencies like the IAEA and the US NRC over the lack of information from Japanese officials on their nuclear crisis is pretty obvious.

 

Listening in on Japan’s NHK World News over the past couple of days, commentators and guests have become increasingly skeptical of government statements on the crisis.

 

 

While the Japanese government have insisted that the radiation danger was localized to within 20 kilometers of the stricken nuclear reactors, Japan’s citizens have watched as the United States, Canada, Great Britain have recommended that their citizens pull back at least 80 kilometers.

 

Indeed, the US State Department is even evacuating non-essential personnel and dependents, and has suggested that Americans consider departing Japan for the time being (see PHE: Advice For American Travelers In Japan).

 

And on Wednesday the US NRC Chairman Gregory Jaczko openly challenged the veracity of some of the information we’ve been getting on the crisis (see Dueling Assessments: NRC Chairman On Nuclear Crisis).

  

These inconsistencies are not likely to escape the notice of the people of Japan.

 

A government’s credibility during a disaster is a fragile thing.

 

It can take years to establish, but require only a few ill-timed or badly crafted statements to damage or destroy.  

 

There is a belief among some government officials that the public `can’t handle the truth’ – particularly in the midst of a crisis - and so their tendency is to minimize bad news or potential threats and emphasize any good news, no matter how trivial.

 

Before the age of the Internet, and 24-hour cable news, that might have worked.  

 

But today – at least in free societies - people have numerous sources of news and information that operate 24/7; everything from Twitter to Facebook to cell phone messaging, and yes . . . even blogs.

 

`Managing’ bad news and information in a crisis is no longer possible, at least not in the long run. 

 

And ham-handed attempts to do so will only undermine a government’s credibility at a time when it needs it the most.

 

Important lessons for all governments around the world to heed, not just Japan.

IAEA Update: Evacuation Status & Fukushima Daiichi Reactor

Amid fresh reports of more problems (see the report at bottom of this blog entry) at one or more Japanese nuclear reactors in the wake of Friday’s devastating 9.0 magnitude earthquake of the coast of Honshu, we get this latest update from the IAEA (International Atomic Energy Agency).

Fukushima Daiichi Nuclear Power Plant

Japanese authorities have reported to the IAEA that Fukushima Daiichi Unit 2 has experienced decreasing coolant levels in the reactor core.  Officials have begun to inject sea water into the reactor to maintain cooling of the reactor core.

 

Sea water injections into Units 1 and 3 were interrupted yesterday due to a low level in a sea water supply reservoir, but sea water injections have now been restored at both units.

 

Evacuation Status

On 12 March, the Japanese Prime Minister ordered the evacuation of residents living within 10 kilometres of the Fukushima Daini nuclear power plant and within 20 kilometres of the Fukushima Daiichi nuclear power plant.  Japan’s Nuclear and Industrial Safety Agency (NISA) has reported that about 185,000 residents had been evacuated from the towns listed below as of 13 March, 17:00 (JST).

Iodine Distribution

Japan has distributed 230,000 units of stable iodine to evacuation centres from the area around Fukushima Daiichi and Fukushima Daini nuclear power plants, according to officials. The iodine has not yet been administered to residents; the distribution is a precautionary measure in the event that this is determined to be necessary.

 

The ingestion of stable iodine can help to prevent the accumulation of radioactive iodine in the thyroid.

Weather forecast

In partnership with the World Meteorological Organization, the IAEA is continuing to monitor weather forecasts and is providing updates to member states.  Since the incident began, winds have been moving away from the Japanese coast to the East, and predictions call for the same patterns to persist for the next three days.

 

The IAEA continues to liaise with the Japanese authorities and is monitoring the situation as it evolves

 

 

 

Meanwhile, NHK World News is reporting that the fuel rods at the Fukushima #2 reactor are fully exposed, and there are fears that a melt-down may be occurring.

 

 

TEPCO:Fuel rods exposed at Fukushima reactor

Tokyo Electric Power Company is battling to cool a reactor to prevent another explosion at its nuclear power plant in quake-hit Fukushima Prefecture.


The utility firm said on Monday afternoon that fuel rods are exposed at the Number Two reactor of its Fukushima Number One plant after the level of coolant water dropped. At around 6:20pm, the power company began pumping in seawater.

 

Tokyo Electric says it had to halt the process due to fuel loss for the pumping system, possibly leaving the fuel rods in the reactor exposed. The firm says a core meltdown might have occurred.

Japan Earthquake Update (13 March 2011 13:55 CET)

Japanese authorities have informed the IAEA’s Incident and Emergency Centre (IEC) that venting of the containment of reactor Unit 3 of the Fukushima Daiichi nuclear power plant started at 9:20AM local Japan time of 13 March through a controlled release of vapour. The operation is intended to lower pressure inside the reactor containment.

 

Subsequently, following the failure of the high pressure injection system and other attempts of cooling the plant, injection of water first and sea water afterwards started. The authorities have informed the IAEA that accumulation of hydrogen is possible.

 

Japanese authorities have also informed the IAEA that the first (i.e., lowest) state of emergency at the Onagawa nuclear power plant has been reported by Tohoku Electric Power Company. The authorities have informed the IAEA that the three reactor units at the Onagawa nuclear power plant are under control.

 

As defined in Article 10 of Japan’s Act on Special Measures Concerning Nuclear Emergency Preparedness, the alert was declared as a consequence of radioactivity readings exceeding allowed levels in the area surrounding the plant. Japanese authorities are investigating the source of radiation.

 

The IAEA has offered its “Good Offices” to Japan to support the nation’s response to the 11 March earthquake and tsunami. One IAEA capability intended to help member states during crises is the Response and Assistance Network (RANET). The network consists of nations that can offer specialized assistance after a radiation incident or emergency. Such assistance is coordinated by the IAEA within the framework of the Assistance Convention.
