Indonesia: H5N1 Clade 2.3.2 Reaches Bali

Not quite a month ago we learned that a `new’ clade of H5N1 had shown up in in Indonesia, causing the deaths of hundreds of thousands of ducks (see Report: Clade 2.3.2 H5N1 Detected In Indonesia) on the island of Java. 

 

Although new to Indonesia, clade 2.3.2 has been circulating across Asia for several years. I wrote of the spread of this emerging clade in 2011 in What Goes Around, Comes Around and EID Journal: H5N1 Branching Out).

 

In the spring of 2010 we began to see reports of poultry vaccine failures in Vietnam due to the spread of a mutated version this clade (further classified as clade 2.3.2.1), which led to this statement FAO Warns On Bird Flu.

 

The H5N1 virus is constantly changing, and as it does, forms distinct genetic groups, or clades. Since its discovery in 1996, the H5N1 virus has evolved into more than 20 distinct clades (and subclades), with many variations within each.

Some of these clades have flared briefly and then died out, supplanted by more biologically `fit’ versions of the virus. But even so there is a good deal of variety among the strains in circulation today.

 

Clade 2.3.2 (and now 2.3.2.1) is very common in South East Asia, clades 2.2.1 and 2.2 are endemic in Egypt and up until recently, Indonesia had only dealt with clades 2.1.1, 2.1.2. and 2.1.3, but now adds 2.3.2.

 

Until this past week, all of the reports we’ve seen of this newly emergent clade in Indonesia have come from the island of Java. 

 

Three days ago, Gert van der Hoek on FluTrackers posted a machine translation of a VOA Indonesia article (New Bird Flu Clade Detected in 3 districts in Bali) indicating the new clade had jumped to the vacation island of Bali.

 

Today, an English Language report from the Jakarta Post, Bali Edition, carries details of this latest discovery.

 

Bali braces for bird flu epidemic

The local health authorities have begun implementing the necessary measures to deal with the ongoing outbreak of avian influenza affecting the duck population, and the possible spillover of the disease into the human population.

 

The measures have been taken following confirmed reports of avian influenza killing thousands of ducks in farming areas in three separate regencies: Buleleng, Tabanan and Klungkung.

 

Laboratory tests have confirmed that the ducks were killed by the new, more malignant strain of the avian influenza virus known as H5N1 Clade 2.3.2. A similar strain of the virus killed hundreds of thousands of ducks across farming regions in Java weeks before the first reported case in Bali.

Although the arrival of this new clade has sparked fears of more human infections, so far, we’ve not seen any sudden increase in reports of human cases in Indonesia.

 

Bali, a popular international tourist destination, has seen more than a half dozen high profile human H5N1 cases over the past several years.

 

In order to protect the island’s economy, in 2005 it was decreed that only locally raised chickens could be sold on the island.

 

But like many other laws and edicts concerning poultry trade in Indonesia, they have gone largely ignored. Ducks fetch a higher price on the island of Bali than on Java, providing ample incentive to smuggle poultry.

 

While it isn’t known whether this new strain arrived via illegal trade, or migratory birds, its arrival has nerves on edge. Bali public health officials have alerted three local hospitals to be ready to receive human cases, and a public awareness campaign is underway.

 

H5N1 remains largely an avian-adapted virus, and for now primarily a threat to poultry.

 

The concern, of course, is that with continual changes to the virus, one of these days it may better adapt to human or mammalian physiology.

 

Which is why, even after 10 years after its return in 2003, we continue to follow developments with keen interest.

Watching Indonesia Again

A hat tip to Treyfish at FluTrackers for picking up a couple of reports this morning regarding a suspected H5N1 case hospitalized at St Elisabeth Hospital, in Semarang.

 

The Jakarta Post has an English language report which provides us with a few details.  The patient is a 54 year old man suffering from a cough, cold, and high fever with (according to another article in Bahasan) exposure to poultry that recently died suddenly.

 

Tests are underway, and so at this time this is just a `suspected case’.  

 

Suspected bird flu victim in Semarang

The Central Java Health Agency says it has received a report of a suspected case of bird flu in the province.

The 54-year-old man, a resident of the Sambiroto subdistrict, was currently under treatment at St. Elisabeth Hospital in Semarang, officials said.

In recent weeks tens of thousands of poultry (primarily ducks) have died across the Indonesian island of Java, and we’ve learned recently of the detection of a `new’ clade of the H5N1 virus (2.3.2) in that country (see Dr. Alan Hampson Interview On Indonesia’s New Bird Flu Clade).

 

All of which serves to put authorities on heightened alert for human infections with the virus.

 

Last week the Indonesian Ministry of Health reported on the death of a 4 year-old boy from the virus, bringing their total number of cases reported for the year to 9 – all of which have proved fatal.

 

There are many other illnesses that can cause `bird-flu-like symptoms’, and so we will have to wait for more definitive test results before we can know if this case is due to the H5N1 virus.

WHO: H5N1 Update

The World Health Organization recently changed the way it reports on human infections with the H5N1 virus with the following notice.

 

Henceforward, WHO will publish information on human cases with H5N1 avian influenza infection on a monthly basis on the Influenza webpage.

 

Cases of human infection with H5N1 will only be reported on Disease Outbreak News for events that are unusual or associated with potential increased risks.

 

Today, we’ve an update that indicates two new infections (1 in Egypt, 1 in Indonesia ) have been reported since the November update (see Latest HAI Risk Assessment Summary 17 December 2012).

Based on previous years’ observations, an increase in reported H5N1 influenza events in poultry is expected for this time of year. Sporadic human cases reported this month in countries with known influenza A(H5N1) virus activity in poultry are within the expected range. No onward sustained human to human transmission was reported.


Public health risk assessment  of  avian  influenza  A(H5N1)  viruses:    The public health risk remains
unchanged. 

 

Neither of these two cases are exactly `news’, in that we saw an Indonesian MOH report on the 4 year-old last week, as well as an FAO report indicated a human case in Egypt.

As you can see from the chart above, increases in human infections with the H5N1 virus typically increase in the fall, and peak during the winter and spring.

 

This report also takes note of the H3N2v infection reported by the CDC (see FluView & A Novel Flu Case In Iowa) in November.  Since then, another case has been reported in Minnesota (see FluView Week 49).


The WHO report concludes by stating:

Overall public health risk assessment of the influenza A(H3N2)v viruses: Further human cases and small clusters may be expected as this virus is circulating in the swine population in the USA. Close monitoring of the situation is warranted.

Due to the nature of constant evolving of influenza viruses, WHO continues to stress the importance of
global monitoring of influenza viruses and recommends all Member States to strengthen routine
influenza surveillance.

 

For now, infections with variant swine flu viruses, and with avian influenza, remain sporadic and only rarely reported.

Indonesia MOH Reports Bird Flu Fatality

My thanks to Gert van der Hoek on FluTrackers for picking up this report from Indonesia’s Ministry of Health, announcing a recent bird flu (H5N1) fatality in West Java.


The following is a machine translation of a press release. Additional information will no doubt be posted on this FluTrackers thread.

 

Report of 192nd Case of Bird Flu


DECEMBER 14, 2012 | 3:56 PM

Ministry of Health, Directorate General of Disease Control and Environmental Health announced a new case of H5N1 has been confirmed by the Center for Basic Biomedical and Health Technologies, Balitbangkes.


Case on behalf of the IT (male, 4 years old) residents of Kampung Nagreg, Gorowong Village, District Parung Panjang, Bogor regency, West Java. Dated 30 November 2012 symptoms of fever, December 1, 2012 the case went to Pustu. Dated December 4, 2012 because there was no change in the case of seeing a private doctor, dated December 5, 2012 morning the case went to the health center and in the afternoon the case was referred to the Private RSIA for hospitalization. Dated December 6, 2012 the case was referred to the Tangerang District Hospital because of fever, cough, and shortness. The situation gets worse case and the case eventually died at 23:40 pm.


Epidemiological investigations have been conducted into the hospital, and the case surrounding the Integrated Team MoH and local health service, possible risk factors that come in direct contact with poultry carcasses (entog) in the neighborhood.

 

With the increase of these cases, the cumulative number of bird flu in Indonesia since 2005 until this news broadcast is 192 cases with 160 deaths.

 

Director General of Disease Control and Environmental Health, Prof. dr. Tjandra Yoga Aditama as the focal point of the International Health Regulations (IHR) has been informed about the case to the WHO.

As Gert helpfully points out in a following post, `Entog’ means `duck’ in Bahasan.

 

Possibly related: we’ve recently seen reports of large die offs of ducks on the island of Java due to the recently arrived strain of H5N1 – clade 2.3.2.1 (see VOA Report On The Indonesian Duck Die Off).

 

This is the first report of a human H5N1 infection since August, although it is certainly possible that some cases are not being picked up or reported by surveillance programs around the world.

 

For now, H5N1 remains primarily a threat to poultry and wild birds, and to a lesser extent people in close contact with them. While human to human transmission transmission has been documented, it is very rare, as the virus remains poorly adapted to human physiology.

 

But around the world more than 600 widely scattered human infections have been reported, and the mortality rate has been a horrendously high 60% among known cases.

 

And so we track these scattered human infections with considerable interest, in case they herald a change in the behavior of the virus.

VOA Report On The Indonesian Duck Die Off

Reports this week that clade 2.3.2 of the H5N1 virus has shown up in Indonesia (where previously clade 2.1 has reigned) have also included stories of large, sudden die offs of ducks (see Report: Clade 2.3.2 H5N1 Detected In Indonesia).

 

Although mortality from H5N1 in ducks isn’t unheard of, ducks - along with other waterfowl - are often able to carry the H5N1 virus without serious ill effect. 

 

In Reservoir Ducks we looked at a 2007 warning from the FAO:

 

FAO warns H5N1 may be lurking in Europe

Oct 26, 2007 (CIDRAP News) – Apparently healthy domestic geese and ducks in Europe may be harboring the H5N1 avian influenza virus, posing a risk to other poultry and to humans who have contact with them, the United Nations Food and Agriculture Organization (FAO) warned in a statement yesterday.

 

In 2009, a leading virologist (see Webster On China's `Silent' Bird Flu Infections) warned in this Reuters Report that:

 

Most types of ducks are not sickened by the virus and in most countries in Asia, they mingle freely with chickens, providing ample opportunity for the virus to jump between species.

 

'The problem is in the ducks in Asia, there is no visible disease in these birds,' said Webster of the St Jude Children's Research Hospital in the United States.

 

The absence of visible infection in poultry makes it harder to track the disease and take preventive measures.

 


All of which makes the sudden death of 300,000 ducks in central Java a bit surprising.  First, a report from VOA NEWS, and then I’ll be back with more.

 

 

Indonesia Identifies New Strain of Bird Flu

In recent weeks, more than 300,000 ducks have died on the densely populated island of Java.

 

The government has since confirmed the deaths were caused by a new and highly pathogenic strain of H5N1, or bird flu.

 

Dr. Rita M. Ridwan, the director of disease control at the Indonesian Health Ministry, says the government is working closely with relevant ministries to investigate further.

 

“So we are in close contact by sharing information, sharing the virus lab and even working together in the field to do field investigations," Ridwan explained.  "I know there are very alarming deaths in the duck population, mostly in the center of duck production by the traditional farming as well as in central Java and East Java.”

While it may seem counterintuitive that a species of waterfowl that normally can carry the H5N1 virus without harm is dying in droves, research has shown that even minor variations in the bird flu virus can greatly affect its pathogenicity.

 


Last April, in Differences In Virulence Between Closely Related H5N1 Strains, we looked at experiments done on mice and ducks with two variants of the same H5N1 clade (2.3.2.1), and the different mortality rates they produced.

 

And among known human H5N1 cases reported to the World Health Organization, in Bangladesh, of the 6 known cases, none have died, while in Cambodia 19 of 21 have succumbed to the virus.  In Egypt, the CFR is running about 35%, while in Indonesia, it is nearly 83%.

 

Although there are likely many factors involved in causing this disparity in CFR – including quality of, and delays in seeking medical care – it suggests that the H5N1 virus is more virulent in some regions of the world than in others.

 

Whatever is driving this higher mortality in ducks is currently a mystery, but is hopefully one that can be solved with a closer analysis of the virus.

Dr. Alan Hampson Interview On Indonesia’s New Bird Flu Clade

On Monday we began to see reports that a previously unseen clade of the H5N1 virus had recently turned up in Indonesia, causing the deaths of tens of thousands of ducks (see Report: Clade 2.3.2 H5N1 Detected In Indonesia) in and around central Java.

 

Today, Radio Australia has an excellent interview (audio & transcript available) with Dr. Alan Hampson about this recent news.

 

Dr. Hampson, who is chair of Australia’s Influenza Specialist Group (ISG), is the former Deputy Director of the Australian World Health Organization Collaborating Centre for Reference and Research on Influenza.

 

Dr. Hampson assesses the risks, discusses how the virus may have arrived in Indonesia,  and defines the difficulties in controlling H5N1 in that country.  

 

He also warns that the H5N1 strains currently circulating in the Middle East appear to be the ones moving quickest towards `being able to become a human influenza virus.’

Indonesia identifies new strain of bird flu: report

The Indonesian Agriculture Ministry has reportedly identified a new, more virulent type of bird flu that's killed hundreds of thousands of ducks in recent weeks.

 

There are suspicions the virus entered Indonesia from other countries - like Vietnam or Thailand.

 

To determine this, the agriculture ministry's veterinary chief has reportedly written a letter to local government offices and the World Health Organisation calling for further research into the origins of the virus.

Report: Clade 2.3.2 H5N1 Detected In Indonesia

Although we tend to talk about H5N1 (aka `bird flu’) as if it were a single entity, in truth, the virus has continually evolved and mutated since its emergence in 1996.

 

You can see the evolution of the virus in the chart below, starting with Clade 0, first detected in 1996.

All of which means we are not watching just one H5N1 virus strain, we are watching at least 20 genetically separate clades of the virus, with many minor variants of each clade thrown in the mix.

 

Different areas of the world have seen different clades set up hen-house keeping, with clade 2.3.2 (and now 2.3.2.1) very common in South East Asia, clades 2.2.1 and 2.2 endemic in Egypt and clades 2.1.1, 2.1.2. and 2.1.3 circulating in Indonesia. 

 

One of the more successful`new’ clades making inroads over the past few years has been 2.3.2.  I wrote of the spread of this emerging clade last year in What Goes Around, Comes Around and EID Journal: H5N1 Branching Out).

 

In the spring of 2010 we began to see reports of poultry vaccine failures in Vietnam due to the spread of a mutated version this clade (further classified as clade 2.3.2.1), which led to this statement FAO Warns On Bird Flu.

 

Today (with a huge hat tip to Gert van der Hoek on FluTrackers for finding and translating), we’ve a report out of Indonesia that indicates that clade 2.3.2 has arrived in that archipelago nation.

 

As noted earlier, Indonesia has been dealing with 2.1 clades of the virus. but today the Director of Animal Health at the Ministry of Agriculture, in a interview for (Bahasan) Radio Australia, has indicated that clade 2.3.2 has been detected in Indonesia this year.

Dr. Pujiatmoko continues to say that they don’t know if this new strain naturally evolved in Indonesia, or was introduced by wild birds or imported poultry.

 

The appearance of this new clade complicates an already difficult situation in Indonesia, although this article doesn’t tell us how widespread this recent arrival may be, or if clade - 2.3.2.1  which appeared in Vietnam in 2010 – has been detected as well.

 

The article goes on to recount recent poultry losses, and provides recommendations for culling infected birds, and the  segregation of ducks (often asymptomatic carriers of H5N1) from other poultry.

 

The bottom line is that the H5N1 virus is a moving target, mutating and evolving, continually looking for an evolutionary advantage. That said, H5N1 remains largely an avian-adapted virus, and for now primarily a threat to poultry.

 

The concern, of course, is that with continual changes to the virus, one of these days it may better adapt to human or mammalian physiology.

 

Which is why, even after 10 years of outbreaks and no pandemic, we continue to follow developments with keen interest.

While recent reports of human infections with the H5N1 virus have been scarce, today we’ve an announcement from the Indonesian Ministry of Health of a recent fatal case involving a 37-year old from Prambanan, Sleman District, Yogyakarta Special Province.

 

Translated from Bahasan, the statement reads:

 

Bird Flu Case report 191

August 9, 2012 | 7:11 pm

Ministry of Health through the Directorate General of Disease Control and Environmental Health announced a new case of H5N1 have been confirmed by the Center for Biomedical and Health Technology base, Balitbangkes.

 

Case on behalf of AW (Female, 37 years) who Prambanan, Sleman District, Yogyakarta Special Province, is a self-employed. Dated July 24, 2012 symptoms of high fever, the following day case treatment to a hospital outpatient PR. Because of persistent high fever and started coughing case, dated July 27, 2012 the case went to the hospital again and finally treated PR. Care hospital in public relations, the more elevated fever accompanied by cough and shortness, so that on July 29, 2012 the case was referred to Hospital B and ventilator. Circumstances of the case gets worse and finally died on July 30, 2012 at 12:05 pm.

 

Epidemiological investigations have been carried into the homes of people and the environment by the Integrated Team Ministry of Health, the local Health Department, Animal Husbandry Department and Officer PDSR Kab.Sleman DIY province, the provincial Agriculture Department DIY, acquired risk factors, namely the possibility of contact with the environment in the workplace and ornamental birds around 50 meters from the house there are cases where the cuts of chicken, quail and duck farms and there is also the case in the home environment.

 

With the increase of these cases, the cumulative number of bird flu in Indonesia since 2005 until this news was broadcast on 191 cases with 159 deaths.

The World Health Organization has just updated their chart of human cases, and has posted this summary of the above report:

 

Avian influenza – situation in Indonesia – update

10 August 2012 - The Ministry of Health of Indonesia has notified WHO of a new case of human infection with avian influenza A(H5N1) virus.

 

The case is a 37 year old male from Yogyakarta province. He developed fever on 24 July 2012, was hospitalized on 27 July and died on 30 July.

 

Epidemiological investigation on the case found that the case had four pet caged birds in his home, which is about 50 metres from a poultry slaughter house and near a farm.

 

Infection with avian influenza A(H5N1) virus was confirmed by the National Institute of Health Research and Development (NIHRD), Ministry of Health and reported to WHO by the National IHR Focal Point.

 

To date, the total number of human influenza A(H5N1) cases in Indonesia is 191 with 159 fatalities, 8 (all fatal) of which occurred in 2012.

 

 

Although we continue to see isolated human infections around the world, and the virus continues to evolve (see H5N1: An Increasingly Complex Family Tree), for now H5N1 is primarily a threat to poultry.

 

The concern, of course, is that over time that could change.

 

And so the world remains at Pre-pandemic Phase III on the H5N1 virus, and we continue to watch for signs that the virus is adapting better to humans.

Indonesian MOH Announces Bird Flu Fatality

The Indonesian Ministry of Health has posted a fairly detailed account of their 190th H5N1 infection, that of an 8 year-old girl from Falkirk District, West Java who died on July 3rd. 

 

 

The girl (initials K.K.) initially developed a fever on June 18th – five days after reported contact with poultry at a live market near her home – and while on holiday in Singapore with her family was diagnosed as having strep throat.

 

She was seen at a local hospital upon her return from Singapore, but appears to have been discharged. Two days later, with her conditioning worsening, she was admitted to a different hospital and then transferred to another facility, where the diagnosis of H5N1 was finally made on June 29th.

On June 24, returned to Jakarta. June 25, the case went to the hospital. Falkirk District B with a complaint over a week heat, vomiting, cough, no appetite and was diagnosed with febrile and impairment of consciousness and no radiographic results Duplex bronchopneumonia.

 

June 26, forced to go home, then went to the hospital. S West Jakarta, the situation gets worse case then mounted ventilator and ICU admission, on June 28, referred to the RSP with the diagnosis of suspected bird flu, on June 29, the results of the sample by the Research and Development (BTDK) Positive H5N1.Kondisi case worsened and finally died world on July 3, 2012 at 22:45 pm.

Today’s report is the first one we’ve seen out of Indonesia since early May.  The apparent CFR (Case Fatality Ratio), based on known cases in Indonesia, remains a sobering 83% (158 fatalities out of 190 cases).

 

The fact that this girl traveled to Singapore while actively infected will no doubt raise new concerns in the region over the ability of this virus to quietly cross borders.

 

While no additional transmission of the virus from this victim has been reported, this is the type of scenario that prompted Singapore to stockpile bird flu vaccine in 2010 (see Singapore Buys 1 Million H5N1 Shots).

 

Although we continue to see isolated human infections around the world, and the virus continues to evolve (see H5N1: An Increasingly Complex Family Tree), for now H5N1 is primarily a threat to poultry.

 

The concern, of course, is that over time that could change.

Indonesian Health Minister Dies

Not unexpectedly, overnight we’ve received news of the death of Indonesian Health Minister Endang Rahayu Sedyaningsih, who tendered her resignation last week due to declining health. She had been under treatment for lung cancer since 2010.

 

Since replacing the logic-challenged former Health Minister Siti Fadilah Supari in 2009, Health Minister Endang signed a virus sharing deal with the World Health Organization and has made small, but visible improvements in her country’s public reporting on H5N1 cases.

Ministry of Health, "Directorate General of Disease Control and Environmental Health" announced one new case of H5N1 have been confirmed by the Center for Biomedical and Health Technology base, Balitbangkes.


Case on behalf of MAF (Female, 2 years) from Siak, Pekanbaru, Riau province.

Dated 17 April 2012 symptoms of fever, dated 20 April 2012 the case to the Private Clinic but no improvement so the evening went to the hospital Z.

On 21 April 2012 cases admitted to hospital and treated in EB Hospital. Treating physician to report to the Command Post Riau Provincial Health Office. On 27 April 2012 patient was referred to hospital AA. The situation gets worse case and the case died on 27 April 2012 at 23:45 pm in the Hospital AA.


Epidemiological investigations have been carried into the homes of people and the environment by the local health office team, acquired risk factors, namely the possibility of contact with poultry (quail eggs) for the case of parents selling quail eggs.


With the increase of these cases, the cumulative number of bird flu in Indonesia since 2005 until this news was broadcast on 189 cases with 157 deaths.

Bali Closes Live Market After Suspected Bird Flu Fatality

Although we continue to see conflicting reports come out of Indonesia regarding the 8 year-old from Bali who is suspected to have died from an H5N1 infection earlier this week (see Bali: Suspected Bird Flu Fatality), today the Jakarta Globe is reporting that a local live market has been closed after the detection of bird flu.

Denpasar. Authorities in Bali closed down the Satria Poultry Market in Denpasar on Friday after several chickens there tested positive for bird flu, days after the resort island recorded the country’s seventh human death this year from the infectious disease.

Anak Agung Gede Bayu Bramasta, the head of the Denpasar livestock agency, said the market would remain closed for at least three weeks.

Bali, a popular international tourist destination, has seen more than a half dozen high profile H5N1 cases over the past several years. In order to protect the island’s economy, in 2005 it was decreed that only locally raised chickens could be sold on the island.

 

But as this story indicates, those rules are routinely flouted and poultry are imported from East Java on a daily basis.

 

If this week’s fatality is eventually confirmed to have been due to the H5N1 virus, it will mark the second bird flu fatality on the island (see WHO Update Mar 1, 2012) this year.  Last fall, a 29-year old mother, and her two children, from Bali died of the virus as well (see WHO Indonesia Update #8 2011).

 

Although we continue to see isolated human infections around the world, and the virus continues to evolve (see H5N1: An Increasingly Complex Family Tree), for now H5N1 is primarily a threat to poultry.

The concern, of course, is that over time that could change.

 

And so the world remains at Pre-pandemic Phase III on the H5N1 virus, and we continue to watch for signs that the virus is adapting better to humans.

Strong Aftershocks Strike Off Sumatra

According to the USGS, several strong aftershocks - including one measuring a remarkable 8.2 magnitude - have struck off the coast of Sumatra over the past couple of hours in the wake of today’s 8.6 magnitude quake.

Overnight the press carried reports (see Indonesia: Sixth H5N1 death this year on Crofsblog) of a new human H5N1 infection out of Indonesia. This time, it involves a 17 year old boy from eastern Lombok island who died on March 9th.

Case on behalf of D (male, 17 years) who Lingsar, West Lombok regency, West Nusa Tenggara Province, is a construction worker. Dated February 28, 2012 symptoms of fever, dated March 1, 2012 cases to the health center outpatient Lingsar, and on March 4, 2012 cases treated at health centers Narmada. On March 7, 2012 cases were referred to the RSU Mataram and treated in hospital. Circumstances of the case got worse and on March 9, 2012 cases died at 23:50 pm.

Avian influenza – situation in Indonesia – update

26 March 2012 - The Ministry of Health of Indonesia has notified WHO of a new case of human infection with avian influenza A(H5N1) virus.

 

The case is a 17 year-old male from Nusa Tenggara Barat Province. He developed fever on 28 February 2012 and sought treatment on 1 March 2012. His condition deteriorated and he was admitted to a referral hospital but he died on 9 March 2012.

 

Epidemiological investigation conducted by a team from the health office indicated that there were sudden poultry die-offs in his neighbourhood.

 

To date, of the 188 cases reported in Indonesia since 2005, 156 have been fatal.

 

 

 

The case fatality rate (CFR) of `known’ cases in Indonesia continues to hover just over 82%, with just 4 of the last 26 cases surviving.

 

What we don’t know (and is highly contested in some scientific circles) is how many cases we are missing.

 

While surveillance and reporting almost certainly misses some number of cases (mild and fatal), researchers have turned up very little compelling evidence to indicate that there are a lot of `mild’ cases going uncounted.

 

Leaving us with a CFR that – while probably artificially high – strongly suggests a significant level of lethality from the H5N1 virus.

 

Fortunately, the virus remains difficult for humans to acquire, as it is presently far better adapted to avian physiology.

 

The concern is (as has been demonstrated as being possible in the laboratory), that over time the virus will pick up the mutations it will need to adapt to a human host and spark a pandemic.

The World Health Organization, in their second GAR (Global Alert & Response) update of the day, has confirmed the Indonesian bird flu fatality reported last week (see Indonesia MOH Announces Bird Flu Fatality).

 

Avian influenza – situation in Indonesia – update

12 March 2012 - The Ministry of Health of Indonesia has notified WHO of a new case of human infection with avian influenza A(H5N1) virus.

 

The case is a 24 year-old female from Bengkulu Province. She developed fever on 23 February 2012 and was hospitalized on the following day. She had breathing difficulty, her condition deteriorated and she died on 1 March 2012.

 

Epidemiological investigation conducted by a team of public health and animal health authorities indicated an exposure to a potentially contaminated environment where sudden deaths of poultry had recently occurred.

 

The case was confirmed by the National Institute of Health Research and Development, Ministry of Health.

 

To date, of the 187 cases reported in Indonesia since 2005, 155 have been fatal.

The Indonesian Ministry of Health has published details on a recent H5N1 fatality in Bengkulu, Bengkulu province on the island of Sumatra.

 

The victim is a 24-year-old female (?) who died on March 1st, who is described as an employee of the District Hospital Dr. M. Jonah. 

 

The epidemiological investigation, however, appears centered on poultry deaths in his neighborhood as opposed to any workplace exposure.

We’ve very few details at this time other than a probable exposure to poultry (hard to escape in rural Indonesia) and the date of her death, March 1st.

Human infections with the H5N1 virus remain, thankfully, rare. The virus is far better adapted to avian physiology than it is to human physiology.

Egypt & Indonesia Confirm New H5N1 Cases

This morning we’ve two reports of human infection from the H5N1 virus, one of which has proved fatal.

 

First stop, is Egypt, where the World Health Organization has announced their 3rd case of 2012, a 1 year-old girl from Gharbeia governorate who is recovering.

 

Avian influenza – situation in Egypt – update

24 February 2012 - The Ministry of Health and Population of Egypt has notified WHO of a new case of human infection with avian influenza A (H5N1) virus.‪

 

The case is a one year-old female from Gharbeia governorate. She developed symptoms on 14 February 2012 and was admitted to a hospital on 15 February 2012, where she received oseltamivir treatment upon admission. She is in good medical condition.

 

Epidemiological investigation into the source of infection is ongoing. Preliminary investigations indicate presence of backyard poultry in her area of residence.

 

The case was confirmed by the Central Public Health Laboratories; a National Influenza Center of the WHO Global Influenza Surveillance Network.

 

Of the 160 cases confirmed to date in Egypt, 55 have been fatal.

Meanwhile, the Indonesian Ministry of Health has posted a notice regarding the bird flu infection, and death, of a 12-year-old boy from Bali.

Case on behalf of DWM (male, 12 years) resident of Badung, Bali. Dated February 11, 2012 fever symptoms develop, see a doctor in private clinics and hospitals. On February 16, 2012 the case was being treated RS. Because of increased shortness, on February 20, 2012 the case was referred to the Referral Hospital Bird Flu, but eventually the case died on February 21, 2012.

 

Epidemiological investigations have been conducted in the home and neighborhood environment cases by the local Health Department with the results of risk factors is unclear.

 

With the increase of these cases, the cumulative number of bird flu in Indonesia since 2005 until this news was broadcast on 186 cases with 154 deaths.

 

Director General of Disease Control and Environmental Health Yoga Aditama Prof.dr.Tjandra as the focal point of the International Health Regulations (IHR) has been informed about the case to the WHO.

 

This is the time of the year when we expect the greatest number of human infections from the H5N1 virus, and so the announcement of four cases this week is hardly unexpected or alarming.

 

The extremely high fatality rate in Indonesia continues (100% in 2012, 83% in 2011), while in Egypt this year, all known cases have survived. 

 

Last year, Egypt’s fatality rate was 38%.

 

The H5N1 virus remains poorly adapted to human physiology, and so far remains primarily a threat to poultry and wild birds.


That could change of course, as the dozens of strains and clades around the world continue to mutate and evolve. So we watch these cases with great interest, looking for any signs that the threat has changed.

Indonesia: Bird Flu Fatality & Suspect Cases

Overnight Indonesia’s Ministry of Health (MOH) announced their 3rd H5N1 fatality of 2012: A 19 year old maid from Tangerang who developed symptoms on February 8th, and died on the 13th.

 

Tangerang is a major population center located just 25 km west of the Capital City, Jakarta.

The epidemiological investigation investing the route of her infection has stated that her risk factors were unclear, suggesting this woman had no known direct contact with poultry.

 

Of note, the  greater Jakarta area has been a bit of a bird flu hotspot these past couple of months.

 

The two other confirmed fatalities this year out of Indonesia (both from January) occurred in nearby Jakarta, while another suspected (but never confirmed) bird flu death was reported in Tangerang on January 21st.

Meanwhile, the newshounds on the flu forums have been following reports of a family of 4 or 5 from South Sulawesi (another recent hub of bird flu reports), that are hospitalized with suspected H5N1.

Since January 1, 2012 there have been media reports of two clusters of suspected H5N1 infections in Gowa Regency in South Sulawesi in Indonesia.

Although one is certainly tempted to suggest that where there is smoke, there is fire, so far none of these `clusters’  has been officially recognized as due to bird flu.

 

Given the limits of surveillance and testing, it is certainly possible that some individual cases, or even small clusters, of H5N1 infection go undetected.

 

But for now the virus remains poorly adapted to humans, and does not appear to transmit easily between them. Human infections remain rare, isolated events.

 

Clusters, while they have been reported, are rarer still.

With 20+ different clades (major versions) of the virus in circulation around the world, the concern is that one of these days one of them will mutate into a form that can transmit well among humans.

Indonesia: Concerns Over Changes In Avian Flu Symptomology

With influenza viruses, the only true constant is change.

 

Viruses must continually evolve and adapt in order to survive. The legacy of infection by a virus is usually some degree of immunity, and if viruses did not evolve they would soon run out of susceptible hosts.

 

Minor mutations happen all of the time, but most turn out to be evolutionary dead-ends. They confer no biological advantage to the virus, fail to thrive, and die out. 

 

It is only when the right combination of changes occurs that changes the virus’s antigenic profile or enhances transmissibility or replication – or expands its host rangewhile still remaining `biologically fit’, that a new strain begins to flourish and spread.

 

And those types of changes can happen very quickly, or they can take years in coming.

 

It is this evolutionary process that has led to having 20+ clades (major versions) of the H5N1 virus circulating around the world, including the vaccine resistant strain clade 2.3.2.1 (see  FAO Warns On Bird Flu) that began to emerge in 2010.

 

 

With so many different clades in circulation, it is no wonder we see some variations in the way the virus behaves.

 

All of which serves as prelude to a long report appearing today in the Indonesian Newspaper Kompas, which talks about a moderation in outward signs of infection among poultry.

 

 

Those with long memories may recall we discussed similar concerns back in 2009 (see Bird Flu Symptoms Changing In Indonesia).  Three years ago the Dean of Faculty of Veterinary Medicine, Institut Pertanian Bogor was quoted as saying:

 

Common signs as cyanosis of wattle, comb or fetlock are not observed anymore. Mortality also decreases so that it’s difficult to differ between ND and AI

 

So while perhaps becoming more common today, these changes do not appear to be entirely new.

Over the years concerns have been voiced that inconsistent or poorly matched poultry vaccination programs could be driving the evolution of the H5N1 virus.

 

As new clades emerge, vaccination of birds with existing vaccines may still lessen morbidity and mortality in the flocks, but not prevent infection.

 

Creating, in a sense, a `stealth' bird flu virus; one that doesn't announce itself with clearly observable symptoms.

 

And that, one would have to assume, only serves to increase the chances that more people and more poultry will be inadvertently exposed to the virus.
